Brian,

Haven't seen an answer to your question yet so I'll give you a place
to start (This URL is no longer valid. It comes up 404 for me.) if
you start by backing up the URL until you get to something that does
work. http://www.customs.ustreas.gov/travel/vessel.htm.

Perhaps someone else will chime in with a current URL or some other
information that will help you better. All else failing, call the
local customs people, if you have any where you are, and ask them the
question. There may be someone who actually answers a phone there or,
they might have something on their phone message about a URL.
