What I think I got out of reading at the FCC site is that for US waters
you don't need a ship radio station license for a pleasure boat if you
only have VHF, radar and EPIRB. When you add SSB, then you need the
license and must list all equipment on the application (not just the
SSB). And you're right that for offshore use you must have a ship radio
station license. It took some digging, but you can find the forms and
the applicable FCC rules at <http://www.fcc.gov/wtb/>

Btw, in addition to each application you submit, you must also include a
form that says how much you're sending and for which application. There
are additional forms referenced on each application that give the cost
for that application. You need to read each application to see what
they want in addition to the completed application. Sure glad we have
the Paperwork Reduction Act in place! :-(
