The biggest practical problem is knowing when to put the harness on.
I have sailed 1000s of mile with out mine on. I put it on when ever I sleep
in the cockpit and when I move around deck. But on long Pacific voyages with
many days in mild weather it is too tempting to leave it off while in the
cockpit.
I know however that freak waves can occur any were any time and I know of
people who have been caught out with big seas in apparently calm weather.

I also would never ever be able to get aboard the boat again after having
miraculous get my hands on the floating line. And I don't know of any that
would manage it. But there are perhaps some Supermen out there who might be
lucky.
The ONLY way to success is: Use them simple proven means available and don't
fall overboard.
