There are circumstances where additional coverage is indicated, such as
Personal Liability where they charge for that of course. Other areas
that may be affected could be in losing a lay-up credit that you'd
otherwise might receive. There is no company that I'm aware of that
surcharges simply for the fact of living aboard without providing some
additional benefit.

WE DON'T EVER, EVER WINK AT THE ISSUE! In a large agency like ours,
where we represent nearly all of the major marine insurers, we simply
get quotes for folks in the companies that we know will accept the risk.

Yes, of course.

Well I've already mentioned Markel and Zurich, both of whom have
specific programs for liveaboards or cruisers, and who will also accept
the same risks in their standard yacht programs.
