I think that you have identified the problem. Most likely the handholds are
through bolted to the deck. The teak plugs over the bolts are probably leaking
allowing water to run down the shaft of the bolts and into the interior
staining the headliner. This is a very common problem with deck mounted fastenings
on older boats. If the problem is fixed early enough the deck core will
probably not be rotted. The cure is simple. Remove the handholds, rebed, and
reinstall using new plugs seated in varnish. If you worry about rot, cover the bottom
of the bolt hole with tape and squirt a bit of Git Rot or thinned epoxy in the
hole. Then reinstall the handholds.
