    I had one, but not in electronic format. I got it free from Boat/U.S. (I'm a
    member). They have a lot of services for folks buying (or selling) boats.
    See: http://www.boatus.com/buyer/, click on "settlement services" for
    information. You'll still have to call them for the form, but they'll
    probably send you one free, even if you're not a member.

    They offer documentation and lien search services, as well as an escrow
    service. I used them when purchasing my current boat (direct from the PO)
    and was very happy with their work.
