You need to fill out form 605, Quick-Form Application for Authorization
in the Ship, Aircraft, Amateur, Restricted and Commercial Operator, and
General Mobile Radio Services. The fee schedule is form 1070Y. Fee is
$150 for 10 years.
Assuming you're operating internationally, you'll also need a restricted
radiotelephone operator's permit, $50 once per lifetime. Also on form
605, although you may have to use two forms.
If your boat is owned by a company vs. an individual, you'll probably
have to get a CORES
registration number before you can send in the remittance advice, form
160. Free.
You'll need to send in form 159, remittance advice with all this.

Nobody said it was easy. As I recall, I had to send in my stuff 2 or 3
times before I finally got it right.
