Here is a link to the FCC form page. Ship station license application is
form 506.
This is a quote from the instructions on the form:
NOTE: You only need to apply if you are required by law or treaty to carry
a radio on your vessel;
or your vessel travels to foreign ports or transmits radio communications
to foreign stations; or
you use marine radio equipment on board your vessel OTHER than marine VHF
radios, EPIRBs,
and radar.
