Sounds like you may have missed putting double clamps on this connection?

Did some research on hose clamps a couple of years ago, and was very
disappointed.
Very difficult to find 316L clamps, most were lower grades of 300 series
stainless.
The operative words are "stain" and "less".
They will rust, it's just a matter of time.
My approach is to double clamp every connection that, if it fails, could
sink the vessel, then plan on
changing out clamps every couple of years.
Just another preventative maintenance item.
