Is this a hydraulic transmission or a manual
transmission? Another way to ask that is, is it a big
fat cable (1/2" or more) that moves a big lever
several inches long, or is it a thin cable (1/4" or
so) that moves a little lever maybe an inch long?

If it's the purely mechanical variety, then I'd
suspect that the cable is out of adjustment. Or that
something is loose in the lever/cable/shift chain that
causes the adjustment to change. Watching the
transmission shift arm (other end of the cable) itself
while someone shifts it might help show you what's
happening. And if someone shifts it into forward and
it doesn't go, then you can always grab the shift arm
and pull on it to see if it goes further and engages
forward gear. That would indicate a cable adjustment
problem or looseness in the cable area.

But it could also be an adjustment in the transmission
itself. Mine is mechanical, and has an adjustment
inside that makes it grab more or less when you put it
    into forward.

If you hold the shift lever in forward, then does it
engage ok? And does it stop going forward when you
let the lever go? Or does it just fail to go forward
sometimes now matter how hard you push the lever into
    forward?

If it is a hydraulic shifter, then there are many more
parts inside that could be responsible for it not
always shifting into forward, or not always staying
there. Have to take it apart and look at seals, etc.

Be sure to check that oil levels are all where they
    should be.
